Worksheet for Visual Art U.S. Copyright Registration

Suggestions and notes: Print and complete a seperate worksheet for each artwork you intend to register, prior to eCo registration. The worksheet is grouped into sections that aid gathering the information, but not necessarily in the same order requested by the eCo. Notes about this worksheet: The U.S. Copyright Office uses the word "author" for all forms of authorship--including art making. For clarity in the context of visual art, this worksheet occassionally uses the term "artist," but this word should be understood to represent the (proper) word "author." Be aware that the questions on this form do not and are not intended to replicate those of the actual registration form or the online version of the form. The purpose of this document is to aid in organizing the information needed to complete the online registration form. The Copyright Office may, over time, make changes to the online registration system. This worksheet is not a publication of the Copyright Office, nor has it been reviewed or approved for use by the U.S. Copyright Office.
